Case Manager/Intervention Specialist (FT 80)
Under the general direction of the Five Oaks Clinic
Director, Case Manager/Intervention Specialist coordinates
the care and service of selected patient populations across
the continuum of illness, promotes effective utilization,
monitors health care resources, and assumes a leadership
role with the multi disciplinary team to achieve optimal
clinical and financial outcomes as well as patient
satisfaction. Additionally, implementing GMH policy and
procedures.
Case Management license required and experience preferred.
